We are a registered provider for Home Care Packages.
If you have more complex needs, you may be able to access a Home Care Package to help you stay at home for longer with coordinated support from South Burnett Care. We offer a range of services to help you to live independently in your own home for as long as you can by directly providing most services or connecting you with someone who can.
South Burnett Care provides you with a co-designed service agreement and an individualised budget, which means you can decide what services you want and when as well as see exactly what funding is available under the package and how those funds are being spent.

If you have not already been assessed by an aged care assessment team for a home care package you need to contact My Aged Care on 1800 200 422. During your phone call you will be asked a series of questions to help understand your needs and find what would best suit your situation.
We will co-design supports with you and deliver services for you so that you can be as independent as possible, feel safe living at home and most importantly so you feel supported to live the life you want. As you change, your support will change with you.
Please be aware, your current provider may charge you an exit fee which will be taken from the funds available in your package. Talk with your current provider to find out if costs apply.
The Australian Government subsidises Home Care Package services, however you may be asked to contribute to the cost of your care if you can afford to do so.
South Burnett Care charges a minimal basic daily fee to all home care package clients, saving you money each week. Our case management fees and administration costs are at the lowest recommended Australian Government rate and we do not charge exit fees if you want to leave us.
Yes, Home Care Package holders can change their provider if they are unhappy with the standard of the service they receive, or if they move.
